Golden Age Foundation Funds Pathways for $25,000
Thanks to support from the Golden Age Foundation, Pathways was gifted $25k to continue providing services to shareholders in need of additional resources and support.
Through their Care Navigation program, they focus on empowering residents to find solutions that will help them remain safe and independent in their homes for as long as possible, while ensuring they have a Care team helping them along the way. This team includes Social Workers, Nursing Students, and Volunteers.

Pathways also steps in to provide support to shareholders who receive a life-limiting diagnosis and don’t know where to turn. They will meet with the family and provide education, support and linkage to the best hospice to meet their needs as well as advocate for the family.
Quarterly, they provide grief support groups on-site at the health center as well as short-term individual grief counseling. Over the past few years Leisure World has experienced much loss and Pathways has given a safe place for them to be with others and learn how to move forward.
And lastly, they have implemented a volunteer program where Leisure World residents are trained and matched with other Leisure World shareholders to assist them and simply be a friend.
All their services are provided at no cost to the family.
If you or your family are interested in finding out more information on their services or how you can volunteer, please contact them at 562/531-3031.
